Here is a little information that will help you understand what to expect, and answer any questions you may have before you visit:
- Please arrive ten to fifteen minutes before your scheduled appointment to complete new patient paperwork.
- Patients who previously completed a MRI or taken x-rays should bring the paper reports or film to their initial exam.
- Children under the age of eighteen years old must be accompanied by a parent or guardian.
- If you have insurance, please bring your ID card with you.
During this visit, you will receive the following:
- Consultation with Dr. Kobata
- Initial chiropractic exam
- X-rays (if needed)
- Treatment
